HP 503 Chronic Disease Care: Evaluation and ImprovementThis course is designed to increase health profession students’ understanding of chronic illness in the U.S., address a variety of problems around chronic illness (e.g., burden on population health, caregiver burden, etc.) and give them the skills to improve the quality of care delivered to patients with chronic illness. The course will emphasize epidemiology, population trends, evidence-based models of care, policy and social-behavioral aspects of chronic disease. Credit(s): 3
